what criterion practices in a safe manner that minimizes risk to patient, self and others

safety

100

a student who requires close supervision 100% of the time, needs constant feedback and simple patients

beginning performance

100

The degree of guidance needed is expected to progress from 100% to being capable of independent performance or with consultation

supervision

100

comments on safety from student or ci?

She practices in a safe manner. She is aware of precautions and establishes safe working environments. She requires occasional cues for body mechanics, guarding and vital signs

CI

100

Where is an expanded narrative found in the CPI?

In the summative comments section

200

name a red flag performance criterion

safety., professional behavior, accountability, communication and clinical reasoning

200

a student who is capable of functioning without guidance or clinical supervision managing patients with simple or complex conditions

entry level

200

as a student progresses through their clinical education  the quality of performance is expected to progress from infrequently to routinely.

Which performance dimension does this describe?

Consistency

200

What is the full caseload at your clinical site for a new graduate?

40-50 patients a week

200

ex/student can evaluate a patient with unstable angina and provide effective educational interventions

this is an example of what performance dimension?

complexity

300

what components of the patient management model are cpi performance criterion

screening, examination, evaluation, dx and px, POC, interventions, all of it!

300

what type of scale is used for the rating anchors

categorical ordered scale

300

A student is at advanced intermediate performance. How much efficiency is the student capable of demonstrating in terms of a FT PT caseload?

A student should be capable of a 75% efficiency of caseload at advanced intermediate

300

What is this an example of?

Initiation of performance of resisted testing in a patient with supra-therapeutic INR despite discussion prior to evaluation re: increased risk for bleeding and clinical reasoning for avoiding resistance testing/activity.Discussion re: plan for evaluation and reasons for avoiding resisted testing occurred immediately prior to entering room.Potential for harm to the patient. CI had to intervene. Upon de-briefing after seeing the patient, student reported that he/she forgot. Strategies to prevent future occurrences discussed including taking notes and asking more questions if needed.

Critical Incident Report

300

What performance criterion is this narrative measuring?

Since the midterm, he/she has continued to follow practice guidelines and demonstrate accountability during patient care and administrative duties. He/She has become more familiar with state, Medicaid, Tricare, and commercial insurance requirements, and she has been independently billing and updating plans of care based on those specifications when appropriate. In her in-service, he/she also included information about telehealth parity laws andBoard of Physical Therapy telehealth guidelines.

Accountability

400

Sample behaviors include: selects relevant information to report, production is accurate, concise, timely and legible, accurately describes the care delivery and justifies skilled services

documentation

400

A student performs with 25% supervision complex patients and independent with simple pts is at what level?

advanced intermediate

400

A student is at advanced beginner level because the student is unable to perform skilled exams, reasoning and interventions.  Simple tasks are performed such as Goni, MMT and a chart review. The student needs to more fully develop which performance dimension?

quality

400
a student who has the potential to demonstrate a behavior or skill but has not had sufficient opportunity, CI has confidence in students ability to perform despite repeated observation, this phrase is reflected in the rating anchor scales

CAPABLE OF

400

A student rates themselves at advanced intermediate for clinical reasoning and a ci rates the student at advanced beginner. Does this indicate harmony in ratings?

No, the student has rated above the level of the CI. A clear discussion of observed pt interactions and self assessment are indicated.

500

Student comments for which criterion?

I have continued seeking out feedback from my CI and from others in the team as appropriate and integrating this to elevate my practice. I have continued touching base with and meeting with my CI regularly to ensure I am meeting our targeted plans and goals. I have continued to participate in interdisciplinary rounds on a daily (or near daily, as schedule demands) basis and am now an active advocate for my patients in these rounds. As mentioned previously, I have had the chance to observe a surgery and have also had the chance to observe a joint replacement education class for upcoming candidates for surgery. This has, effectively, meant that I have seen some patients's course of care from their pre-operative education to the time of discharge from acute care therapy. I have also continued to be involved in the professional development/quality improvement projects my CI has spearheaded at the hospital (wound care, orthopedics) and feel that these have been essential to my ongoing professional development and understanding of the change process in health care

Professional Development

500
what level is this student at?

capable of maintaining 50% a therapists caseload

proficient with simple tasks and only developing consistency in skilled exam and reasoning

needs supervision 75% time with complex pts

intermediate performance

500

the levels of tasks , pt management, and environment increase with ci controlling fewer of the elements

complexity

500

A CI states they never rate students at full entry level because of the complexity of the practice setting, for ex/ICU. The student performs at advanced intermediate on safety. Will the student not pass the final internship?

It is likely if only one performance criterion is not at entry level and the comments of the ci are not congruent with the rating, the program clinical education faculty will assign the student grade, in this case the student will be successful given the ci comments

500

student assesses functional arom, bed mobility, and interviews patient to determine if they are ready to transfer from bed to chair.

is this a sufficient screen and exam to perform the assessment and intervention of functional mobility ?

No patient safety indicates further testing, discussion with team, and data on patients physiological responses to movement,
