Completion of collateral contacts, Completion of SDM assessment of risk and validity determination, Completion of all investigation activities, notification, and documentation for foster home,  restrictive care facility, and day care   report type investigations.  When there is a valid finding for abuse/neglect, the Form 10 shall be generated and sent to the District Attorney upon closure of these types of investigations. If there is a CINC proceeding, the Form 10 can be generated prior to closure of the investigation and sent to the District Attorney.
