Some healthcare professionals might be mandated to report their suspicions of a patient's domestic violence victimization. What are two advantages and two disadvantages of mandatory reporting? Finally, what is YOUR opinion of it?

This is a highly controversial topic, as it puts the doctors and nurses in a precarious position. 

Some potential advantages include improving the numbers in regard to data collection for the prevalence and context of intimate partner violence/domestic violence, along with an increased likelihood that the abuser will face criminal prosecution. Additionally, mandatory reporting will encourage all healthcare personnel to readily monitor the signs of IPV and will help them to identify cases of abuse. 

Some potential disadvantages, however, include placing the victim at an increased risk of harm, taking autonomy and power away from the victim, and it could compromise doctor-patient confidentiality. For these reasons, it might deter victims from reporting cases of IPV to their doctors.

What is the problem of high-risk offenders? In other words, what are these individuals like? Why might it be hard to prevent them from engaging in intimate partner homicide?

Unlike most batterers, high-risk offenders for committing intimate partner homicide (IPH) often go undetected by law enforcement. These offenders tend to be male, middle-aged, married or recently separated, and they conform to social expectations (i.e. they hold a job, have a family). However, they typically do not engage in repetitive IPV and do not tend to have a criminal history. These individuals are especially problematic because they tend to explode when they are threatened with separation or when they experience extreme jealousy, prompting them to commit IPH. Unfortunately, it is especially hard for law enforcement and advocates to detect these offenders before they commit homicide.

Describe one type of secondary consequence of abuse (i.e. either medical problems, behavioral problems, mental health problems, or Battered Women Syndrome). *Provide details about the impact it has on victims.

Medical problems: 

-Women with IPV injuries tend to present minor injuries, but they display health problems at a rate 60 times higher than their peers. Thus, they are significantly more likely to visit the emergency room or their primary physician. 

-They have an increased prevalence of gynecological problems, including an increased risk of STI's and HIV. 


Behavioral problems:

-These women are more likely to fear child abuse, suffer from depression, and rely on drugs and alcohol to self-medicate. Even worse, they are 15 times as likely to abuse alcohol and at the extreme end, they are five times more likely to attempt suicide. 


Mental health:

-After being exposed to a pattern of abusive behaviors and psychological warfare, victims of IPV are substantially more likely to suffer from a range of mental health disorders. To start, their risk of suffering from a mental health disorder after being exposed to abuse increases by 500% and at least one in ten of these women suffer from a psychotic break. Of course, many of these victims suffer from Post Traumatic Stress Disorder (PTSD) and this is especially true when they have been stalked and/or sexually assaulted. 

-These women are likely to develop Battered Women Syndrome, which predisposes them to learned helplessness. This makes it hard for them to recognize or access resources that might help them escape their situation in a law-abiding manner. Even more, once these victims are exposed to two cycles of IPV, they are predisposed to this syndrome. It exacerbates their risk of depression and in numerous cases, leads to violent acts of self-defense against their abusers. This has been used as a defense in criminal cases.
