An assessment of any injury as performed by a member of the medical staff. Incidents not related to a donation such as a donor slipping on ice outside the center, a donor catching his hand in a door, etc. should be reported via an Incident Report rather than the DAE Report. If in doubt as to which reporting process to use, the Corporate Safety, Health, Environment Officer or Medical Operations personnel can provide consultation.

NO. If the event is reported by the donor after the date of occurrence and the event is resolved, it is not necessary to conduct a follow up contact with the donor

Leave it blank, but add a comment as to why no vitals were taken. IE: EMS took donor out of building, etc.

FYI: We can ask EMS for vitals they have taken to record them.
