What is MassHealth?
Need areas assessed in the SNA
What are self-help skills, sensorimotor skills, communication skills, independent living skills,affective development skills, social development skills, behavior development skills and wellness?
Must have ID or DD and need Day Hab to acquire, improve or retain their maximum skill level and independent functioning.
Must be written in measurable terms and address a specific skill acquisition and retention
What is an objective?
A clinical assessment tool that is completed based on the results of the Service Needs Assessment.
What is the Day Hab Leveling Tool?
Must contain information necessary to identify the program member.
What is a member record?
The degree to which individuals can perform daily living activities and manage their lives independently.
What is Functional Level?
Emergency evacuations, search plan, bomb threat, gas leak, and medical emergencies.
What are Emergency Policy and Procedures?
Must be completed within 5 business days after members admission
What is the Interim DHSP
Team that consists of a RN, developmental specialist, program director and the following: Speech Therapist, OT, PT and Behavior specialist if applicable.
What is an Interdisciplinary Team? (IDT)
Updated monthly.
What is a progress note?
Impacts one or more areas of a member's health status.
What is significant change?
Specifies the level of payment for the service and as applicable the medically necessary units of ISS.
