therapist is in charge of telling Eli what their coping skill bag is.

Have conversations with Eli on discharge
as soon as its confirmed, email "residential discharges" group, and start planning (usually want at least 30 days to plan)

How do you determine discharge for ERMHS
Therapist and treatment team will still state and recommend when a youth is ready for discharge.
Discharge date MUST be confirmed in IEP with school district.
AAP: between therapist and parent
CPS: therapist (and treatment team) will recommend that youth is ready for discharge, CPS team will then find placement.
probation: therapist (and treatment team) will recommend that youth is ready for discharge, probation will agree/disagree. Official discharge date will be determined by probation, therapist and caregiver (if they have one). probation can also be dual status CPS too.

As early as possible, tell kaiser liaison that is the plan. turn in referral ASAP. Normally need at least 30 days prior to discharge for these referrals.
For PHP: tell team as early as possible: ask what their procedure is for their region
How to figure out therapy after discharge?
AAP: work with case manager on what they had before, what insurance they want to use, if they have Medi-Cal, etc. AAP is eligible for WRAP services
CPS: will do this for you: all youth that discharge from STRTP are required to have WRAP type services
Probation: kinda the same for CPS: double check with probation on their specific process for their region
What are ways youth discharge unexpectedly?

when is the Summitview discharge summary due and who is it sent to
BEFORE the youth leaves. IT IS MANDATORY TO GET YOUTH AND GAURDIAN SIGNATURE prior to discharge. You can start this form 30 days before discharge, and complete with confirmed facts/dates week of discharge.
Summitview Discharge summary is given to caregiver, youth (if they want a copy) and the referring party
how is discharge date determined
based on therapist, parent and kaiser liaison. Kaiser look at if the youth needs this level of care and medical necessity.
coordinate with case manager to determine youth's insurance. Can use what they have used for therapy in the past.
if Medi-Cal: can utilize their open access line to start services again.
what forms need to be completed
IF they are EDC billable youth (the youth eli makes you do the service verification forms for) you will do the EDC discharge packet (CFT minutes, CANS, PSC35, 3MChild-Tay, KET, summitview discharge summary, county discharge summary)
If they are NOt EDC billable youth, only the normal forms for all, and any additional forms eli tells you about
What needs to be confirmed for youth prior do discharge
follow up mental health appointment (psychiatry and therapy)
school is figured out
safety and plan for home (expectations etc)
any additional needs (did they have any medical issues, etc)
What meetings/coordination needs to have happened prior to discharge
CFT to make sure all agree with discharge to create plan, then final CFT prior to discharge to confirm plan
conversation with liaison to make sure kaiser is in agreement with discharge (if needed, usually only needed for complex cases)
final family sessions to make sure plan for home (safety plan, expectations, agreements, etc) have been discussed.
What meetings/coordination needs to have happened prior to discharge
CFT with team to determine all agree youth is ready for discharge
IEP (at least 6 weeks out) to confirm with school district discharge and discharge plan.
Final CFT or IEP right before discharge to confirm plan.
final family sessions to make sure plan for home (safety plan, expectations, agreements, etc) have been discussed.
when is EDC packet due?
5 days after youth discharges
