What is incomplete documentation and missed care planning follow-up?
A new plan must generally be completed within this many days after PNCMA/PCMA completion.

What is a specific obligation/due date?
This person's information should be used when the member cannot answer due to age, capacity, or circumstances.

What is the work list?
A note says "PNCMA completed" but does not mention needs, barriers, next steps, or IMP updates. Identify the issue.
What is incomplete documentation?
If a PNCMA identifies food insecurity, transportation issues, or caregiver stress, the IMP should be updated with this.

This date tells when the next required contact or follow-up must occur and should not be guessed.
