Who is your Coordinator?

This is the Trinity Staff Member to whom you should report possible abuse, neglect, self-harm, safety concerns, and overall job communications.

What is the payroll policy?

If documentation is not completed (submitted, corrected, AND signed) by the 7th of the following month the mentor will not be paid until the 28th. If documentation is not completed within 30 days from the date of service, the mentor will not receive any payment.

What should be documented in a Team Meeting progress note?

The Mentor's contribution to the meeting, progress made towards the consumer's Mentoring goals, and the Consumer's input (if possible), and who is in attendance.

What is proper progress note language?

Correct answers include: Full sentences, Strength-based language, Rephrasing your narrative into factual observations that can be put into documentation, such as "This Mentor prompted..."

What are appropriate telehealth activities?

This includes, but is not limited to, arts and crafts, board games, reading a book, puzzles, limited gaming and YouTube videos, or any other activity that can be tied to a recovery goal.

What is the expectation for frequency of client contact?

The mentor should have weekly contact, including documentation when contact could not be made.

How is documentation time recorded?

The Mentor should include the exact number of minutes it took to write the progress note in their documentation.

What is the expectation for attending team meetings?

The Mentoring Coordinator should request the Service Facilitator include the Trinity Mentor in team meetings. If the Mentor attends, they should include this on their time sheet. If the Mentor is not invited to attend, they should contact their Mentoring Coordinator.

What is the purpose of the "How did the client respond?" section of the Progress Note?

This portion should include a quote in the client's own words to show that the client is actively engaged and responsive to the service you are providing. If there is no quote, this section must include a direct observation.

When is it appropriate to bill for travel time to/from a telehealth appointment?

When making a no-contact drop off to deliver necessary supplies to the consumer to be used during the telehealth session.

When is an appropriate time to request your Coordinator attend and/or discuss a team meeting with you?

If you are discussing changes in services with the client's team, if you cannot attend, or if you need further support.

What is a cancellation note?

This is required documentation when a meeting does not occur or is rescheduled by either the client or the mentor.

When should telehealth services be offered in place of in-person services?

Telehealth is a temporary solution to be offered to clients when in-person services are not available.

What is your job responsibility?

Your job is to work on the contracted Mentoring goals provided to you in a fun and creative way, and document progress made towards those goals. The Mentor does not work for the client's care givers or service facilitators. The Mentor reports to their Coordinator (your first point of contact) and the Administrative Staff.

What parts of the service plan apply to the Mentor?

The Mentor is to adhere to the authorized number of hours and the recovery goals set forth by the service facilitator.

What is the purpose of a team meeting?

The opportunity to give each member of the consumer's team a chance to provide an update on progress towards recovery goals, and make changes to the service plan.

What is the Individual Service Plan, Recovery Plan, or Outcome Sheet?

This is the legal document provided by the Service Facilitator laying out the clients' goals to be worked on with the Trinity Mentor.
