Based on the referral form, why did Duane’s teacher refer him for an evaluation?

His teacher referred Duane because he is having difficulty listening in the classroom. He can’t seem to remember all of the information. This could be for directions if they are reading out loud, or watching information via video. Then he either can’t complete his work or doesn’t follow directions, and has to ask others. It is becoming disruptive to the class. He also seems to have trouble understanding vocabulary words and information from the textbooks and stories. His sentences are often grammatically incorrect.

It would be helpful to see a portfolio assessment of his work in class and to hear more from teachers about his behaviors, mood, motivation, and attention span.

You could provide behavior-specific praise like, “Thank you for sitting in your chair and working hard; you are staying so focused!” or more general comments such as, “Great work, we’ll take a break after 5 more questions!”
