How should the discussion guides be used?
A. To ensure they are addressing all the bullets that pertain to the case, the supervisor and specialist should both have a copy of the guide during the discussion to reference.
Q. When a case moves from out of home to In Home, what guides do we use?
A. The placement of the child will determine what guides are utilized. If the child is in the home, the in-home guides will be utilized.
What does the date at the top of each discussion (date completed) on the Supervision Documentation Tool mean
The date of the discussion.
Q. What do I document in the documentation tool?
A. Select the appropriate button for each line (Yes, No, or N/A), then sign and upload the form. The follow-up text box can be used to capture follow-up actions, although this feature will not be in Guardian version 1.1. Supervisors and Specialists are encouraged to explore alternate ways to manage cases and tasks outside of this.
Q. When are the discussions supposed to occur?
A. The conversations are designed to be pro-active and held at or before key decisions. Refer to Policy for guidance on timeframes.
Q. When should the sufficient information discussion guide be used?
A. Any time after initial contact when there is more to be learned about the family in order to make an informed conclusion about impending danger.
Where does the signed tool land after uploading it to a case note?
Hard file
Q. What does it mean to “Sign off” on the documentation tool?
A. The supervisor’s signature is validating that everything has been entered with quality in the electronic record or case file.
Q. Does the Transfer Dialogue count as a discussion?
In Progress Update Discussion Conditions for Return are discussed how often?
At minimum monthly.

Q. When does the documentation tool get uploaded?
A. At a minimum, prior to the end of the investigation and after every discussion in Case Management. It can also be done after each conversation.
Q. Do cases need to be discussed every 30 days in Ongoing and/or In Home?
A. Yes, cases need to be discussed monthly at a minimum
How does AHIT response impact the investigation process, for the receiving investigator?
The receiving investigator would review the documentation provided by AHIT and pick up where AHIT left off
Can the case planning tool be used more than once?
No, it is only for the initial case plan. Use the Progress update tool for reassessing case plans.
Q. What type of case note do I attach the Documentation tool to?
A. A SUP/MGT case note in CHILDS at this time.
Q. If In-Home or On-going get a report they need to investigate, would they use the Investigations Discussion Guides?
A. Yes.
In ongoing, if you have one child in the home and one child out of the home, would you utilize two different discussion guides (progress update) every 30days?
No, they can use the out-of-home discussion guide. Some topics don't apply to the child in the home, therefore, they don't need to talk about those things. We don't require Specialists to document separate C-CSRAs because one child is in home.
Do you complete the Investigations Documentation tool for anything other than a report? What about when you close out an action request, or court ordered investigation?
If a safety assessment needs to be made the supervision process, to include the documentation tool, would be used. If this is not needed document in a case note/note in Guardian
Q. True or False: Action items on the Documentation Tool (yes/no) should be reviewed during the clinical discussion.
A. False; the supervisor should be reviewing the case prior to the clinical when possible. The no’s should be discussed with the specialist to develop and action plan and target dates for completion.
