No your start date should be 04/03/2022 and the end date should be 90 days from 04/03/2022 making it 07/02/2022.

While writing my quarterly I noticed that John Cena has achieved Goal 2: John Cena will improve his ability to appropriately express when he is upset, whats next? 

Your overall progress area should address that the goal has been achieved and also state the new goal that you will be changing it to. Also, your target date should be the end date.

My client has shown slight progress when comes to Goal 1 John Cena will improve his emotional regulation, should I state that in my overall progress area even though I marked the goal status as ongoing?

No! Refrain from mentioning any type of improvement shown if you're keeping the goal as ongoing.

What are the 3 errors found in this strategies section: John Cena and his caregiver and/or legal guardian will learn and implement at least 5 out of 10 strategies weekly to assist and support him with managing his emotional needs within the next three months of his participation in skills training and development service as needed.

Revised, because any edits needed in a quarterly are to be addressed in the overall progress area. The actual change will be made in your ISP

What is the difference between the ISP Summary of Presenting Needs Including Psychiatric, Medical and Social and the Quarterly summary of case mangement services?

Summary of case management services discuss the resources given to the guardian over the past 90days of services. 

Summary of Presenting Needs Including Psychiatric, Medical and Social discuss the client’s current challenges across three areas: psychiatric, medical, and social needs. It ensures that each identified issue is linked to appropriate interventions or referrals for support. (sports, therapy, and more).

What is wrong with this overall progress area for an ongoing goal: The client still needs help learning coping skills that works for him, The client resonates with skills he can hear or be hands on with like creating a "positive word" jar. rather than other things. The client hasn't had any physically aggressive episodes as of recent, but he still struggles with getting upset when things doesn't go his way leading to silence or a very irritable attitude. The goal will benefit from ongoing.

1. Never refer to the client as "the client" in a quarterly or ISP.

2. No need to discuss what the client resonates with as this is just for discussing the clients struggles with the objectives.

3. If its ongoing nothing about the client showing signs of progress should be stated.

What are three key elements that must be included in the "Summary of Case Management Services Provided" section

The frequency of services (consistent or inconsistent), how long services were provided (full 90 days or less), and the specific names of resources or organizations linked to the guardian.

What is the purpose of the "Strategies/Frequency of Services/Persons Responsible" section in an ISP?

To specify how often services will be provided, who is responsible for providing them, and how interventions will support the client’s goals using structured resources.

I realize that my clients guardian has only received two case management resources in the past 90days, how do I complete my summary of case management?

Simply state that services provided were inconsistent and discuss the two services provided. Strive to provide the adequate amount of resources now that you're the new QMHP for the client!

What is discharge plan and discharge criteria? Give an example of each.

Discharge plan is tailored specifically to the client while criteria is the same no matter who the client is. 

Criteria: The client may be discharged if they have met all their goals for three consecutive quarters, no longer meet the Level of Care (LOC) requirements, move out of the service area, and if the guardian request

Plan: Personalized plan detailing the next steps for the client after discharge to ensure continuity of care outside of AOC. Just because they're no longer with AOC that doesn't mean we dont want them to do great in life.

What are some errors found in this challenges section: John continues to struggle with managing defiance, improving emotional regulation, and reducing reactivity. John is also showing difficulty with redirecting anger constructively, using coping strategies and often projects his anger onto others. John needs to work on behavior management, emotional control, and express his emotions. John would benefit from reating better channels for expressing his emotions,such as through art or journaling, will empower him to communicate his feelings in a healthier manner, ultimately fostering stronger connections with peers and promoting a more positive environment.
