EI Treatment Plan signature Page, EI Eligibility Determination (if initial), EI Service Grid. 

The Helpdesk scans them in.

What's the difference between a planned start date and actual start date on a service grid (NOT service coordination)? 

A planned start date is 30 days out from the date of signed consent. The actual start date is the first visit date (or can be the date of signed consent IF strategies were given on that day).

  • We are a voluntary program and you can choose to end services at any time.
  • You have the right to request or refuse an evaluation or team meeting at any time.
  • You have the right to disagree with recommendations, informally and formally.
  • You have the right to an interpreter. 
  • Child’s records are confidential, but you have a right to access them. 
  • EI is a free service (If you receive a bill DON’T pay it and let someone on your team know)
