Are AHA's present in the field & signed?
AHAs are present in the field, reviewed monthly at job-wide safety standdowns and modified when needed. STA based off the AHA is performed daily.
Crisis Management Plan & Emergency Action Plan
Two mock drills were conducted. Several medical responses on the site. Base Emergency services conducted confined space & elevator training (Delaware) for their entire staff. Both drills had a review process to find failure points and implement change. Were then reviewed in the staff meetings.
Safety Management Tool
SMT is reviewed daily in morning staff meetings. There is a SMT for each jobsite. Two items are addressed daily. Site specific items were added. SMT is addressed in PPE reviews.
Safety Audits
(Hensel Phelps)
Audit teams have been selected to pair more experienced team members with those with less experience. Teams walk twice a week, and it has been turned into a competition with the winning team being rewarded and recognized in staff meetings. Safety managers have monthly walks with all AS's to discuss upcoming risks.
Confined Spaces
All confined spaces are marked on the project. Confined space permits are filled out daily and de-classified with continuous monitoring required. Jobsite confined space board is used in the main trailer with a log sheet. ECB3 also has the first MSA monitor system used to track individuals live in the field with live alerts via text messages and live readings from the office and phones.
Does AHA content address High Risks?
The AHA's are reviewed and modifications occur after Near Misses & Accidents. Its also reviewed at Prep meetings & initial inspections as well as when out of sequence work is identified.
Material Handling Plans
Every floor and general site has a material handling plan with a laydown area. Site access is addressed. Several hundred S hooks were purchased and given to trade partners for cord management. A composite clean up crew was also established and cleans multiple times a week. Site plans are used for coordination in the trade partner meetings.
Near Misses & Lessons Learned
5 near misses investigated to 8 accidents. All incidents discussed in trade partner meetings, all hand safety meetings, & staff meetings. Trade partner executives are brought to the site for serious incidents to review. Lessons learned are clearly communicated and APP and new items such as Scissor lifts permits have been created.
Respiratory Protection
ECB3 is a dust free jobsite. Sweeping compound is provided and Zambonis used when needed. All trades are following table 3 Silica controls to eliminate the need for respirator use. Hensel Phelps has all Hilti tools with the HEPA Vac systems.
Public Exposure
Public exposure is controlled. The gate at our trailer is closed to vehicle traffic for two entry points to the parking garage for pedestrians only which keep workers out of vehicle paths. Main employee lot has a separate walk path protected from the haul road. When work was being conducted on NSA a Public Exposure Audit was being conducted weekly. In terms of the bridge on NSA a pedestrian walk path was established to keep NSA employees away from the jobsite and a traffic control plan was established and implemented.
Was an STA performed (Hensel Phelps)
Hensel Phelps performs STA's daily with inclusion of the energy wheel. The STA form has also been updated for jobsite specific items. Trade partners are also performing STA's

Executive Safety Charrettes
ESC are held monthly with major trade partners (Southland & MC Dean). additionally, ESC are held with trade partners to discuss accidents and production goals where safety is always discussed in depth first. Example CAS.
Cranes & Third Party Inspections
All cranes receive a third-party inspection and load test prior to operation. Crane submittal is reviewed by both Hensel Phelps and USACE with all documents provided including: operator cards, rigging cards, load charts, ground pressure, lift plan set up, etc. Area superintendents are present at every inspection and verify with the safety manager that the submitted plan is met. Wind conditions are monitored.
Excavation Permit
Excavation permits are field out weekly along with daily trench inspections. All permits have attached updated underground utility drawings and high-risk areas are hand dug or hydrovaccd. Additionally, Gen yard hazards now also require hand placement of stone or digging due to energization of the switch gear. Excavations are also protected from vehicle exposure via concrete or water filled barriers and fall protection is required via the AHA after 6' in depth.
Competent persons identified in the AHA?
All AHA's have competent persons identified. in the submittal to USACE proof of competency is attached. Additionally, a component person form is submitted by their company executives. Trade partners are submitting completed inspections.
SAFE Participation
OM & GS talk with staff on the importance of completing Safe cards. Card count board is established in trailer lobby. Top performers receive gift cards monthly in staff meetings for recognition. Foreman participate. Trends are identified and used in trade partner meetings. SAFE pilot program established for 12 people. New way to look at SAFE.
CARES
CARES is conducted monthly with a 1-1.5 hr jobsite walk followed by a luncheon discussion, review, & resolution portion. All trade partners are involved. CARES members are given identifiable vests and a budget. Also given rewards for participation. Findings & solutions talked about in all hands safety meetings and trade partner meetings.
Riggers
Rigging certifications are validated on all crane plans. Rigging cards are provided for any AHA that requires rigging in the field. Rigging inspections are done monthly after the all hands safety meeting. Hensel Phelps has had 4 Maryland Rigger 1 classes here on site for our staff. Red vests that say Rigger are used for identification in the field.
Fall Protection
A site fall rescue team (Hensel Phelps) is established and trains quarterly. No other jobsite in the district has done this. A fall rescue scenario was also practice with the base fire department. Fall protection is required for excavations over 6' in depth.
Accident Prevention Plan
The APP is kept up to date. Modifications we made to adhere to USACE jobsite requirements. Additional updates were made in response to Accidents or Near Misses such as the Scissor Lift Permit. It is reviewed with the trade partners in the foreman's meetings and sent to them via email.
Foreman Indoctrinations
Foreman indoctrinations are conducted prior to the start of work and yearly for all foreman. Performance is reviewed for Hensel Phelps foreman. All foreman are recognized when going above and beyond for safety through golden ticket program.
Trade Partner Safety Audits
Trade partners with safety managers are conducting and turning in weekly safety inspections. Trade partner leadership also participate in Hensel Phelps Safety Audits. All site safety managers have monthly safety meetings and walks.
Operator Certifications
All operator certifications are received via the submittal process with the AHA prior to the start of work. a operator matrix was developed using excel to track current Boom & Scissor lifts operators with a 3 year expiration date. Trade partners for heavy equipment must submitted a document authorizing their operators. All Hensel Phelps staff are tracked on a separate excel sheet and have operator cards provided including for Heavy Equipment.
Hot Work Permits
A Monthly hot work permit is issued from USACE and daily hot work permits are required for all trade partners signed off by an Area Superintendent or higher only. Fire watch requirements are 1 hr in length. Hot work permit was changed with an additional check list added for this jobsite only.
