How to make an Incident Report

What is:

Incident Reports are completed on Procare. You can find the Incident button when you click on “Daily Activities” 


  1. When filling out “Nature of Injury/Incident”, describe what the injury is (ex. Joe has a pink scratch ¼ inch long on their left cheek. No blood present.) Next, describe what happened to cause the injury and include where staff was located  (Joe was playing near another peer in the block area and took peer’s toy. Peer scratched Joe’s left cheek. Staff was stationed in the block area, and immediately intervened.) Note: Do not include any identifying information about the other child (no pronouns, names, etc) However, in the event that another peer was involved (scratching, biting, hitting) that child will also need a separate Incident Report created for them.  

  2. When filling out “First Aid Administered”, describe all actions you took to help the child. (ex. Staff comforted Joe and washed Joe’s scratch with soap and water. Staff attempted to apply a band-aid, but Joe could not tolerate the band-aid and took it off.) If the incident was a bite, ice must be applied ASAP. 

  3. Next you will enter in the staff member present (likely the person who is filling out the report.

  4. You will sign. This can be done on the online report. 

  5. Click the “parent was notified” button. Procare will send the parent’s the report, 

  6. once the ELC Leadership team has approved the report. Notify the ELC Leadership Team of the incident report. They will read it, approve it, sign it, print it off, and deliver the paper copy to the classroom. 

  7. Have the parent/guardian sign the Incident Report at pick-up. In the case that the child rides the bus, it will be sent with the bus aide, signed by the parent/guardian, and then brought back to the center. 

  8. Turn in the Incident Report to the Incident Report Folder located in the Staff Mailboxes.

Steps to using the ELC Dishwasher

What is:

  1. Switch the dishwasher switch to on. It will need to heat up before use.

  2. Once it is hot, load the basket with dirty dishes.

  3. Place basket in the dishwasher

  4. Hit the Start button, and let it run its cycle

  5. Open and unload, placing everything in it’s correct spot. 

  6. Periodically, dump the dishwasher’s food trap.

Description of how staff use transition sheets at the ELC.

- Anytime a child arrives in our class, we write down their name. 

- When a child leaves the class (going home, using bathroom during recess, pulled out for therapy) we highlight their name (*There is a spot to check if they are going to ECSE)

- Anytime the class physically transitions to a new place (outside, new classroom, etc) do a name to face and check mark the child on the transition sheet

- When a staff member comes to a classroom to be counted as ratio they write down their name and do a name to face check of all of the children they are responsible for.

- When a staff member leaves a classroom they highlight their name

- When classrooms combine, place transition sheets together, and communicate/to a name to face check of all of the children in your care.
