Counseling Session, Verbal Warning, Written Warning, Final Written Warning, Suspension, Termination. 

No, it does not have to be in order. The level of warning is determined by the infraction and/or any previous warnings the employee received.

Yes! Any/all documents presented to employees become legal documents once presented.
