When the weather is nice, may I wear open-toed shoes?
No, most likely not. Any employee that works or does business within a medical clinic must wear closed-toed shoes at all times. This includes all PCC clinics, PCC behavioral health clinic, complementary medicine, pediatrics, audiology, physical therapy, radiology, internal medicine, pharmacy, dental, optometry and all clinics within the hospital. This is outlined in the SCF Dress and Personal Appearance Procedure.

How are you educated about safety?
All staff receives safety education during New Hire Orientation and during annual reorientation. Periodic education occurs in clinics when mock codes are conducted, tracers are completed, drills are run, environment of care tours are conducted, etc.

All programs should have an emergency phone tree that can be activated in emergency situations. All staff should understand the process your program has in place. The emergency phone line that employees can call for updated information and announcements is 729-KISS (5477).

What is the SCF policy about wearing name badges? Why is it important that we wear our badges and keep them visible?
Employees must wear the badge when entering or leaving customer areas while performing their work or work related activities. a.The employee’s picture, name and job title must be clearly visible at all times. b.Pins or any other items may not be worn or attached on the badge. SCF security officers will check employees for identification badges and submit monthly tallies to the Safety Manager or Safety Officer.

How do staff know a room has been sanitized properly and equipment has been sterilized? How would a staff member inform a surveyor that a room is clean or an item is sterilized?
Staff must be able to verbalize the process for cleaning customer-owner care areas. They also must be able to explain the process for sterilizing equipment (who sterilizes them, how often, etc.). Also, if there are check-off sheets that are used, those must be completed routinely as outlined in the policy and on the sheet.

Can you clearly identify what items meet the definition of regulated medical waste?
Soaked with blood or other potentially infectious material. Regulated medical waste is also known as “biohazardous waste,” materials that are contaminated by blood of body fluids. If the material is clearly soaked in regulated medical waste it needs to be disposed of in a biohazard container.

Biohazard bags, no matter their size, must only be used for biohazard materials/items. Clean items/supplies cannot be stored in clear biohazard marked bags. Once items are placed inside these bags, they must be considered as biohazard and be managed and disposed of appropriately.

The 2015 National Patient Safety Goals (NPSG) are: Goal 1: Improve the accuracy of patient identification. Goal 2: Improve the effectiveness of communication among caregivers. Goal 3. Improve the safety of using medications. Goal 6. Reduce the harm associated with clinical alarm systems. Goal 7. Reduce the risk of health care-associated infections. Goal 15. The organization identifies safety risks inherent in its patient population. There is a 2015 flier that should be visible and accessible to all staff and c-os. Most clinics hang a copy in each exam room. Please make sure this information is easily viewable to everyone.

An unfamiliar individual is walking around in your clinic. You are busy working on a project. Is it okay to ignore them?
Any time an unfamiliar individual is in your working environment, stop what you are doing and introduce yourself. Assure the individual has a SCF escort with them. If not, politely ask who they are and what agency they work with. Then ask them if you can assist them. We do not want individuals (ie. external surveyors) walking around the clinics without escorts. Also, all visitors should be wearing proper identification from their agency and/or a visitor’s badge.

When speaking to a surveyor, both SCF staff and ANTHC staff should state that they work for Alaska Native Medical Center (ANMC). SCF and ANTHC are united as ANMC.

What is the Disaster Manual Quick Reference?
The Disaster Manual Quick Reference is a red-trifold (blue in non TJC accredited programs) that covers how to respond to natural disasters and codes. Each program should have at least one of these tri-folds that is easily accessible to staff. Staff should know where their program’s tri-fold is and what information is inside.

Who from your Program should be prepared to speak to surveyors during a The Joint Commission survey? What things should they have prepared? What if they do not know the answers?
Everyone should be prepared to speak to a surveyor. Surveyors tend to seek out staff that are “on the floor” and work directly with customer-owners. When you are performing your job every day and following policies and procedures, you are preparing yourself for a survey. Stay current on changes in your program and ask questions when new processes are introduced. Educate yourself on standards and participate in program activities (such as tracers) that include The Joint Commission education. If you do not know the answer to a question a surveyor asks you, do NOT make something up. The surveyors understand that staff may not know all the answers (and they know staff can get nervous and intimated); let the surveyor know that you are unsure about the answer to the question and will either get back to the surveyor with the correct answer, or will lead the surveyor to another individual with the answer. Make sure to follow-through with what you say.

What is an SDS (previously MSDS) binder and what does it provide?
The Safety Data Sheet binder that has information designed to provide both workers and emergency personnel with the proper procedures for handling or working with a particular substance. SDS's include information such as physical data (melting point, boiling point, flash point etc.), toxicity, health effects, first aid, reactivity, storage, disposal, protective equipment, and spill/leak procedures. These are of particular use if a spill or other accident occurs.
