At the end of internship, Dr. Padilla started therapy with a psychologist who worked at the agency where she did her internship. Dr. Padilla never had this psychologist in any evaluative capacity during her time as a student and therapy was in this psychologist's private practice. The therapeutic relationship ended in 2014, after a total of 5 sessions (post-doc got very busy). In 2015, Dr. Padilla left for the mainland. In 2019, she returned to work at the agency as a provider, alongside the other psychologist. As part of being colleagues, as well as having children the exact same age, Dr. Padilla and the other psychologist developed a friendship in 2020 due predominantly to the close living proximity and isolation created by the pandemic. What is/are the ethical consideration(s)?
Multiple Relationship
While it is clear that a multiple relationship occurred at several points of this scenario, this poses a minimal ethical risk. First, the potential for exploitation of me being a student was nil, as the other psychologist never had nor will never have evaluative authority. Second, my privacy as the patient was protected by seeing the other psychologist in their private practice, as opposed to the agency where they worked. Third, as a member of a small professional community, there is always potential for patients to be/become colleagues and we are entitled to quality care. Lastly, even though this was not a sexual relationship, the 2 year principle could be applied. The friendship started 6 years after the termination of care.

After consulting with several colleagues prior to this meeting, it was determined that this was an ethical and appropriate research method. No identifying information was collected and all participants were given the same parameters. An argument can be made that Dr. Padilla's presence while taking the survey may have led to undue influence/pressure to complete the form, as she will be assisting the team with sport consultation. However, it was determined that the benefits of the information acquired by the survey outweighed the risk of such influence.

While working in a hospital setting, Dr. Padilla was called to consult with medical staff for a patient they believed lacked capacity to consent. The medical doctor requested that Dr. Padilla evaluate on site to determine capacity to consent to medical treatment. The patient presented with psychosis and had a septic infection in two of her fingers. The medical doctor felt it necessary to amputate.
