We volume at the program volume screen to allow the opportunity to update the volume if it was programmed incorrectly.  Proceeding beyond that screen prevents us from being able to edit that volume.

Ask the donor for the full name and last 4 of SSN.  Compare the name and SSN to the DIC and label set.  Verify the donor matches the photo printed on the DIC.  Verify the control number affixed to the donor card matches the control number listed on your label set.

Screening the wrong donor (Identity).  Label swap (Identity).  Underdraw/Overdraw (Safety).  E&A (Quality).  Retraining and annoying CAPAs.
