What is an additional correspondence recipient?
This is someone that is granted limited authority to assist the applicant. They cannot complete an application/review or request service(s).
What is a facilitator?
This role can act in place of the member/consumer. Multiple roles on the chart are entered as this admin role.
What is a medical representative?
This is an individual or corporation who is appointed by a court to act on behalf of an individual.
What is a guardian/conservator?
Release forms are not required for this role, but they must express involvement in the consumer’s case. Entered as a med rep.
