What is the difference between an Incident Report and Injury Report? Who can write either or?
Injury report is any injury to any child which occurs during the hours while such child is in care, and which requires hospitalization or emergency medical treatment etc.
Incident report is a record of any unusual or serious incidents including but not limited to behavioral incidents, injuries, property destruction or emergencies.
Staff have 24 hours to document the incident.
When Site Director conducts ongoing performance observations, what is it called and how often is it done?
All Direct Care Staff will be observed by his or her supervisor every other month. FKO uses an assessment tool called Arnett that will be filled out by the supervisor to complete observations every 2 month. After completion, the supervisor will review the observation with the staff member and will remain confidential. Every observation will be kept in the employee's staff file.
If a child communicates to you concerning information, what steps do you have to take? and why?
First step is gathering as much information as you can. Making sure we are not promising the child you will keep it a secret.
