The risk outcome classification for a client stating that she has had a previous abortion but is not considering abortion at all for this pregnancy.

If a client has ever had an abortion she will be classified as AV because she has done it in the past.
This procedure should be followed if you want to request to take a planned PTO day.

Fill out a PTO request sheet, submit it to Alondra. She will confirm you have PTO available and send to your supervisor for approval. (Alondra will notify you of approval.)

We receive a request from the Ct's doctor asking for a copy of the client's sonogram. The client has a signed release in her file allowing us to fax her records for medical purposes, our medical team has signed it but Dr Z's report is not in the file can the report be faxed to the client's doctor?

The client's sonogram record is considered incomplete without the doctor's interpretation. We can call Dr Z and have him do a speedier sign off if needed.
Our center is funded in one of these ways.
