This form is filled out every time a new medication is picked up for a foster youth

Foster Parents Must Schedule These Two Appointments for the youth Within 30 Days of Placement
What is Well-check and Dental Exam?
This housing program is available to foster youth to support with the transition out of foster care and into independent living
What is Transitional Housing Program?
This form is completed monthly to document each time a foster youth takes prescribed, OTC, or PRN medications
What is a medication administration record (MAR)?
This is the medical insurance that every foster youth qualifies for in SLO county
What is Cen-cal? (AKA Medi-cal)
This person is someone that the foster parent knows and trusts, to provide respite, babysitting, and overnight care. It is the foster parent's responsibility to find this person.
What is a Temporary Care Provider?
Foster parents must complete this with foster youth within 30 days of placement to ensure youth understand what to do in an emergency situation
What is an emergency drill?
Foster parents must report any unusual incidents (illness, accidents, injuries, dangerous behavior, and medication errors) as soon as possible to this team member
What is FCNI Social Worker?
This form is completed monthly to document each time a foster youth takes a psychotropic medication
What is a psychotropic medication administration record (P MAR)?
This practice technique used by social workers involves planning for reunification and an alternate permanent placement at the same time, in case reunification doesn’t happen.
What is Concurrent Planning?
This form must be completed each time a foster youth has a medical appointment
What is a Health Encounter Form?
This is developed at the initial CFT meeting and is considered the "primary document" that guides all services and treatment
