FALSE: We can request eval/IFSP records or get them from parents/guardians, but a child will need a new intake and eligbility evaluation to determine qualification in WA state.

TRUE OR FALSE: A family wants to decline services before age 3. Can I offer a re-evaluation?

TRUE! It is highly encouraged to offer a re-eval (DAYC, Battelle, or HELP etc.) before a family discharges from services to ensure child is meeting developmental milestones. Familys can decline this as well, we just need to document it was offered!

A family is returning to HopeSparks. They had an initial IFSP with us back in June 2024. What does the FRC/provider need to do next?

FRC: Mini intake + HELP checklist (an annual review is due next- last IFSP was in June 2024). Send to case rounds for provider assignments. Complete Annual review paperwork and update Credible Plan with new outcomes and services.

Provider: Assigned therapist/Developmental Specialist joins FRC for an Annual Review IFSP meeting. Complete annual enrollment paperwork. Schedule service evaluations for 1st visits (if applicable). 10 business day offerings and 30 day start date timeline will apply.

If a family wishes to exit services, what is needed from a family in order for the FRC to close out a file?

Decline ESIT services paperwork (signed), Exit COS (if applicable), and County Change-ESIT form (send to DDA).

I have an Initial IFSP due on June 18th 2025. What date do I need to call the family by to offer 2 dates for the IFSP?

June 3rd. We do not count the IFSP due date in the 10 business day timeline (so really, it's like 11 days!). Important to keep in mind for Holidays too!
