What is Validation form to clear pre-requisites. If the student needs help filling out from walk-in list to have counselor assist.

I am trying to get into Math-25 but it says I have not met the pre-requisites. But I took the EAP test and they said I could go straight into Math-25.

What is a valadation form. Please put on walk-in list so counselor can varify the test score and assist with form. Student must have a copy of EAP scores to attach to form.

What is validation form for the chem-10 pre-requisite but student will still need to take Bio-20 or its equivalent to be able to register into Bio-22. Please see counselor on a walk-in basis for further information.

What is challenge form and test in Modern language center, unless student completed AP spanish then they would need offical test scores on file and a walk-in with a counselor to determine level.

I got a letter in the mail that said I needed to speak to counseling because I am on probation. *Student does not know what level they are how would you find out the level and what is the procedure for levels 1-3.

What is using SACS on Datatel. If level 1 student can be assisted on a walk-in basis with our counselors or the success guides in the success center. Level 2 and dismissal must go through opening doors. Provide student with phone number 909-652-6201.

What is all sealed transcripts must be submitted to admissions and records. After transcripts have been submitted student my fill out a pre-requisite valadation form in counseling. Needs 24-36 hours to scan transcripts to be assisted on a walk-in basis with a counselor.

What is they can see a counselor on a walk-in basis. Please be advised this is a time sensitive program and if we are not offering the information or counseling meetings for ME first the student cannot participate in the program.

I am trying to register for classes and it says I have restrictions. *After looking student up in PERC you see they have a C1 block. What does this mean and how does the student remove it?

What is a high school student. In order to remove a C1 block student must take the assessment test and have completed a high school orientation (HS SEP). It can also be removed if student is 18 or older. Please see Nancy or Dixie to remove block.

I am trying to do the Nursing program and the check off list says my next step is to do an external transcript evaluation.

What is a transcript evaluation form for health sciences. Please make sure all transcripts from other colleges have been turned in prior to submitting. Student must include ALL colleges attended including Chaffey.
