What is the standard complete process from beginning to end of an enrollment from a CarePort referral?

Receive the referral, pull information up in Cerner, enter referral into Apricot, connect with patient, update referral and Apricot notes, accept CarePort referral, update referral status in Apricot, enter enrollment documents and note, enter Cerner enrollment note, enter outgoing referrals, complete SDOH assessment and update demographics.


Contact client for a check in post discharge and schedule HV, enter note into Apricot.

Conduct HV, enter note and HV assessment into Apricot and enter HV note in Cerner. 

Connect with client for a check in/follow up and schedule follow up HV if needed, enter notes in Apricot.

If no other needs, change status to closed as needs met. Enter Exit Information in Program Enrollment.

When you run your reports and it shows that you have pending documentation for your referral note. When you click into the file, it doesn't show that anything is pending. What should you do?

Think about if you ever changed the note from a referral to an enrollment. Check the note type, if it says "Enrollment", change it back to referral and the option should pop back up at the bottom of the note, showing your pending status. Change that status back to "Please Select" and change the note type back to "Enrollment" and save the note. Double check your documentation.
