What is Medicaid?
This document must be signed by the participant or Authorized Representative (AR) at every certification and out-of-state transfer

This is the position the participant's hand must be in when performing a hemoglobin test to ensure proper blood flow

WIC participants must provide this for proof of participation in adjunctively eligible programs
What is documentation of eligibility, such as a certification letter?
This signature must be collected before taking anthropometric measurements and/or conducting a hemoglobin screening
What is Consent?
This participant category must be screened and/or referred for immunization status at certification and mid-certification appointments
What is Infants?
When measuring the weight of an infant, they should be dressed in this way for accuracy
What is a dry diaper and undershirt?
This must be done if air bubbles are visible inside the cuvette after filling it with blood
What is discarding the cuvette and using a new one?
TANF/SNAP cards are only valid as proof of participation if this information is included
What are current eligiblity dates?
This screen in HANDS is where the "Consent" signature may be found
What is the Medical Screen?
These participant categories must be screened for lead testing and substance use
What are infants, children and women?
This should be removed before taking the length measurement of an infant to ensure accuracy
What are shoes, hats and hair accessories?
This should be done with the first 2-3 drops of blood before filling the cuvette
What is wiping them away?
This practice is not allowed for determining adjunctive eligibility for WIC certification
What is self-declaration?

This participant category requires a referral for lead screening
What is Children?
If the first two length measurements differ by more than 1/4 inch, this is the next step
What is taking a third measurement and averaging the two closest?
This part of the skin must be thoroughly dried after cleaning with alcohol to avoid dilution of the blood sample
What is the puncture site?
These three programs provide automatic eligibility for WIC

The Rights & Obligations must be presented to the participant in this manner to ensure understanding
