DS-3053 is used for what applicates...

The DS-3053 is the preferred document to provide parental consent for a minor. Although a statement of parental consent may be acceptable in lieu of the DS-3053, if the statement does not contain all of the required information, processing of the minor's passport may be delayed.

The DS-3053 is used only for obtaining the notarized consent from the non-applying parent or legal guardian. The DS-3053 requires more information on the non-applying parent or legal guardian and must be notarized. It also includes additional clarification on form requirements, such as the necessity for the notary to witness the signature in-person.

The DS-3053 is submitted with the Forms - DS-11 Application for a U.S. Passport for any minor child under the age of 16 when a parent cannot be present.

Form DS-3053 is to be submitted by the non-applying parent or guardian to provide written consent for their minor child to apply for a U.S. passport because they are unable to appear in person with their child.

When completing Form DS-3053, the customer is to be advised to make sure that ALL the information is complete, correct and in black ink.

DS-5525 is used for what applicates...

The DS-5525 is used only if the notarized, written consent of a parent or legal guardian with custody of the minor applicant under 16 cannot be obtained.
