1. The names of the victim and suspect and the relationship between them. 

2. Descriptions of the demeanor of the victim and suspect. 

3. Any spontaneous or excited utterances (res gestae) statements made by any party. 

4. Interviews with any witness or possible witness in the household, including children. 

5. Description of any destruction or disarray, or lack thereof, at the scene. 

6. Any visible or reported injuries (e.g., location and description). 

7. The date, time, and location of the incident(s) and any known prior history of family violence. 

8. Disposition of the incident including the facts leading to the decision not to make an arrest. 

9. The on-duty field supervisor's name the officer contacted, if required by policy. 

10. An alternate telephone number of a close relative that will remain in contact with the victim. 

11. The presence of any firearm in the household or at the incident location. 

12. That an officer provided the victim with a Blue Card

13. Any notification reference numbers (e.g., Department of Family and Protective Services).

If an incident warrants an officer making an arrest for Resisting Arrest, Search, or Transportation, _________________ , or Evading Arrest or Detention, the arresting officer shall notify a supervisor prior to contacting the appropriate intake office or filing charges.
