Look at Guide for Immunizations, enter into CP, ask HNC to check (for Hib)
At what age is a Dental exam a requirement?
3 years old and older
When do you complete an IHR?
Anytime a child is diagnosed chronic condition or when a concern arises.
What health events do FA/FCCS enter into CP?
IZ's, Health Letters, Growth assessments
What form do you give parents at enrollment when they are missing health requirements?
Missing Health Requirement... Health Letter 2 weeks before it is due if needed.
Why is it important to enter Health Letter event in CP?
For documentation of follow-up and so that we can see notes and updates.
What health and nutrition forms do you complete with parents at enrollment.
Health History, Nutrition Assessment, CACFP non-pricing letter, (if applicable: Food Sub, Medication Consent form)
How often should you review IHR events to update as necessary?
at least monthly.
When do you need a SCP and who should be in that meeting?
When child has special care needs in the classroom. Parent, FA, teaching staff, CD.
When is a TB test needed?
When Doctor determines the child is "at risk"
What age do we require blood pressure?
Children over 3 years old.
When do you complete Sunscreen Permission Slip?
At parent request.
When would you complete an IHR for hearing and vision?
If child had 2 failed attempted or 4 uncooperative attempts at hearing and vison.
What form needs to be completed before medication is administer to a child at site?
Medication Administration Consent Form.
What HN requirements can you complete at site if they were not completed at last WB/physical?
Hearing and vision observations/ screening (if certified); Growth assessment (ht and wt); Blood pressure
At what age is Lead required and when do you complete IHR for it?
12 and 24 month WB. When lead is 4.5 or higher.
What forms need to be kept at site for licensing purposes?
Immunizations, WB/Physical, TB
What should you review for information that might require IHR?
Child's application, WB/Physical, Health History, Nutrition Assessment, information shared by parent.
When does a food Substitution Not need to be signed by medical provider?
When it is for personal beliefs, dairy intolerance, or vegetarian diet.
When does a child need to be excluded for having a missing Health Requirement?
When it's a licensing requirement: Up-to-date immunizations; TB risk assessment/ test (if needed); Current WB/Physical (30 of enrollment)
At what WB is a Hgb test required and when do you complete IHR for it?
12 month WB; if Hgb is 10.9 or less
When is the Center Exposure Notice posted and the Exposure to Infectious Diseases notice given to parents?
When it's a confirmed that the child has a communicable disease (not COVID)
What are some common conditions that require IHR?
Asthma, Dental, Lead, HGB, Life threatening food allergies, seasonal allergies, hearing and vision concerns.
What do you need to do when a child has seasonal allergies?
Get doc. note, complete SAP with parent, additional forms, add to list in classroom/review with teaching staff, send to HNC.
