CC: SOB. A&Ox4. BP stable. CTA on exam.
The patient came in because they’re having trouble breathing. They’re awake and fully aware of who they are, where they are, the time, and what’s going on. Their blood pressure is normal, and their lungs sound clear on exam.
Pt with LOC, EMS brought in. GCS 14, CT head neg. No acute findings.
The patient briefly lost consciousness and was brought to the hospital by emergency services. They are awake and mostly alert now, though not completely back to normal. A CT scan of their head didn’t show any serious or urgent problems.
CC: CP. EKG wnl, trops neg. CTA lungs. Low suspicion for PE. D/c home with precautions.
The patient came in with chest pain. The heart tests were normal and didn’t show signs of a heart attack, and their lungs sounded clear on exam. We don’t think this is a blood clot in the lungs. They’re safe to go home, but should come back if the pain gets worse, they have trouble breathing, or develop any new concerning symptoms.
Pt with calf pain, r/o DVT. US pending. NPO for possible procedure.
The patient has pain in their calf, and we’re checking for a possible blood clot in the leg. An ultrasound test is still pending to look for this. They shouldn’t eat or drink right now in case they need a procedure.
Pt with hx DVT presents with SOB. R/o PE. Placed on 2L NC. EKG wnl. Remains stable. DNR status noted. Admit for monitoring.
The patient has a history of blood clots and came in with shortness of breath. We’re checking to see if there might be a blood clot in the lungs. They were given a small amount of oxygen through a nasal tube to help with breathing. Their heart test was normal, and their condition is stable right now. They have chosen not to receive resuscitation if their heart or breathing stops. They will be admitted to the hospital so we can monitor them closely.
EKG wnl, labs unremarkable. NTGA on abd exam. D/c home, f/u PCP, return PRN. DNR status confirmed.
The heart test was normal, and the lab results didn’t show anything concerning. The belly exam didn’t show any pain or abnormal findings. The patient is being sent home, should follow up with their regular doctor, and should come back if they feel worse or have new symptoms. They have also confirmed that they do not want resuscitation if their heart or breathing stops.
