How often should staff be documenting behavioral data? 

Daily. Staff should be documenting all behaviors observed on the behavior data sheets.

If no behaviors occured, then 0 or no behaviors should be documented on the behavior data sheets

What is a precursor behavior? What are examples of precursor behaviors?

A Precursor behavior is any response that tends to occur immediately prior to a challenging behavior 

Examples: Rocking body back and forth, pacing, yelling, individual talking to themselves.

Why is it important to use positive communication with the individuals? 

Using a authorative/parental tone is a form of abuse and should not be used with adult individuals. A more positive communication approach is more effective to develop a positive relationship with the individuals.

Why is the behavior documentation so important? 

This information is important to be used in audits/investigations to ensure staff are supporting the individuals effectively during challenging behaviors.

To support the BIS with advocating medication changes, level of supervision changes etc.

If a individual has eloped from the building without staff supervision, What interventions can the staff use to redirect the individual? 

If an individual is running into moving traffic and is compromising their safety, staff will call 911. Staff can use restrictive interventions such as SCIP-R 2 person escort to redirect the individual to safety.

If the interventions in the behavior plan are not effective and the individual is in a crisis situation and endangering their safety and others, how can staff intervene? 

Calling 911, Use emergency SCIP-R interventions to keep individual safe. Notify RN,BIS and supervisor for further instructions.

If an individual's behavior has changed or new behaviors are being observed, what can the staff do to support the individual. 

Contact the nurse/BIS/supervisor. Keep the individual safe by redirecting them into a quiet safe enviornment, using SCIP-R interventions to ensure their safety.  Documenting all behaviors and interventions on the ABC chart and TLOG.

As soon as the incident is over or has to be completed within a 24 hours

If an individual is engaging in behaviors which are risking their safety and others but SCIP-R interventions are not in their Behavior Plan? How are staff supposed to intervene? 

Staff will use SCIP-R interventions in an emergency situation to keep the individuals safe. Staff will then notify Supervisor and BIS of the incident and what SCIP-R intervention was used. Staff will document on the GER the behaviors observed and indicate the SCIP-R intervention used.

When an individual is in a crisis situation and the assigned staff needs support? What can the team do? 

Call 911, Remove other peers in the room, push furniture away from individual, assist with SCIP-R interventions, assist with getting protective equipment per the behavior plan.

What are examples of coping skills staff can use to redirect challenging behaviiors? 

Deep Breathing techniques, counting to 10, going for a walk, listening to music/watching videos/ going into a quiet enviornment, reading a social story, and exercising.

Behavior information (type of behavior, frequency,intensity, and duration)
