Blood glucose <70 or symptomatic with other adult present and cause known apply the 15/15 rule. Once blood glucose is above 70, then feed patient a carb + protein snack, if it is time for a meal then have a patient eat a balanced meal.

CHW is to advise home care if:

BP>200/120 & no cardiac or neurologic symptoms, and is not on any blood pressure medications.

BP>180/110 & missed most recent dose of blood pressure medication.

CHW should call AMN now if:

Meat, fish, shrimp, eggs, seeds and nuts,  tofu.

Patients blood glucose >240 & uses insulin. (drink one glass of water/hr for the next 4hrs. take insulin/meds as directed.
Patients blood glucose >240 & does not use insulin: treat drink at least one glass of water/for the next 4hrs take meds as directed.

CHW is to advise patient home care if:

When blood sugar less than 70, treat consuming with 15gms of carb. Wait 15min, then after pt consumes recheck blood sugar. Then have patient eat a snack or balanced meal.

Ex of sugar options to treat: 3 (4g)glucose tablets, 4oz of fruit juice, 5-6oz of regular soda(not diet), 7-8 gummy or regular life savers, 1tbsp of sugar or jelly.
