What is Level of Detail in Coding?

What is a Combination Code that includes symptoms?
Pneumonia, Acute bronchitis, Lower respiratory infection, Acute respiratory distress syndrome, and Acute respiratory failure
What are common respiratory manifestations of COVID-19?
These codes cover situations when the initial treatment of a disease has been performed and the patient requires continued care during the healing or recovery phase, or for the long-term consequences of the disease.
What are Aftercare codes?
Characters for categories, subcategories and codes may be either a letter or a number.
What is the Tabular List?
The residual effect (condition produced) after the acute phase of an illness or injury has terminated.
What is a sequela?
When using one of these combination codes, an additional code should not be assigned for the symptom.
What are Combination codes that include symptoms?
These codes are assigned when individuals who previously had COVID-19 and are being seen for follow-up evaluation, and COVID-19 test results are negative.
What are codes Z09 and Z86.19?
Codes in this category, Encounter for supervision of normal pregnancy, are always first listed and are not to be used with any other code from the OB chapter.
What is code Z34?
This abbreviation in the Alphabetic Index represents “other specified.”
What is NEC?
Additional codes for manifestations that are not an integral part of the disease process may also be assigned when the condition does not have a unique code.
What are Syndromes?
These are combination codes that include the
substance that was taken as well as the intent.
What are Codes in categories T36-T65?
The provider documents "suspected," "possible,"
"probable," or “inconclusive” COVID-19.
What are Signs and symptoms without definitive diagnosis of COVID-19?
These codes are never permitted to be used as first listed or principal diagnosis codes.
What are “In diseases classified elsewhere” codes?
A note used to define and/or give examples of the content of a particular category or a block of category codes.
What are Includes Notes?
Use of code X36.0- is limited to collapses of man-made structures due to earth surface movements, not due to storm surges directly from a hurricane.
What are Sequencing of External Causes of Morbidity Codes?
These codes correspond to the sequence of events regardless of which caused the most serious injury.
What are combination external cause codes?
Codes from Chapter 15 always take sequencing priority.
What is COVID-19 infection in pregnancy, childbirth, and the puerperium?
These codes are to be first listed, followed by the diagnosis code when a patient’s encounter is solely to
receive radiation therapy, chemotherapy, or immunotherapy for the treatment of a neoplasm.

For a newborn that tests positive for COVID-19 and the provider documents the condition was contracted in utero or during the birth process.

These codes are used to indicate the surveillance of a condition that is fully healed and no longer exists.
