What is a student with a disability and who requires specially designed instruction?
This team member usually sees the student five (5) days a week. They know best how the student is doing in a classroom setting.
What is the regular education teacher?
This meeting is held once a year within 365 days from the previous IEP.
What is the annual review?
An individualized instructional program for eligible students with disabilities that are provided beyond the regular school year.

Who are the IEP meeting participants?
I support the decisions made by the IEP team and secure resources.
What are the responsibilities of the LEA?
This meeting determines if one or more of the following criteria are met: 1) have a physical or mental impairment that substantially limits one or more major life activities, 2) have a record of such an impairment or 3) be regarded as having such an impairment.
What is a 504 plan?
It guides special education and regular education teachers in determining the effectiveness of their instructional procedures. Provides information for writing present levels and progress reports.
What is progress monitoring data?
I am completed at least every 3 years, required before dismissing a student from special education, must be completed within a 60 day timeline, and must include a discussion of all relevent data about the student.
What is a reevaluation?
This person is responsible for making sure that all members are following the IEP, oftentimes providing various levels of educational support.
What is the special education teacher?
This meeting is scheduled to address necessary changes to the IEP. It may be held at any time during the IEP cycle.
What is an addendum?
Teachers must sign this document verifying that accommodations were provided during local and state testing.
What is the review of accommodations (ROA) form?
Classroom and district or state assessments

This person is responsible for testing and evaluating the student.
Who is the school psychologist?
This meeting is held when the psychologist is ready to share his/her findings.

This person must agree and sign the IEP prior to the provision of services.
