- Appears dazed or stunned

- is unsure and forgets instructions in class or game

- Moves clumsily

- answers questions slowly

- can't recall events prior of after hit/fall

- shows mood/ behavior/ personality changes

  1. Ask the student what happened? 

  2. Fill out Student Incident Report Form 

  3. Submit the incident Report form to your coordinator to submit on Informed K12
