If there are DX but not on common DX list, I will need to google for at least 1 code.

Care Transition Coordinator

They create and develop the community relations to convince other companies to refer to SCH. They also can be intermediary between these community partners and our diverse SCH teams. Furthermore, they work hand in hand with clinical to schedule and coordinate their referrals during working hours with potential to assist afterhours.

How is facility used at SCH? The referral tab has 2 ways of describing facility, touch on both. 

1. Facility is used to describe where the person is living, such as ALF, ILF, SNF. 


2. Facility is used in the referral tab and within SCH sometimes as a way to describe what company is the referral source, such as hospital, clinic, community resource, ALF.

When does HCHB fail to upload the PDF?

When the pdf is 10MB of data or more. 


This can be confusing as many assume it's due to # of pages.
