Two types of assessment steps for skin.
What are inspection and palpation?
This is a result of inadequate circulating blood, frequently seen in high stress states.
What is pallor?
Less than 1cm, flat different color than the rest of the skin. Examples include freckles, petechiae and moles
What are Macules?
Elevated, has pus in cavity. Example is acne.
What is a pustule?
This makes up about 50% of dust in your home.
What are dead skin cells?
Check this beneath the clavicle.
What is skin turgor?
Skin redness associated with a variety of rashes and other conditions.
What is erythema?
Less than 1cm, elevated, solid. Examples include warts and skin tags
What are papules?
Irregular area of edema on the skin. Examples are insect bites, hives and allergic reactions.
What are wheals?
Skin contains 11 miles of these.
What are blood vessels?
The presence of excess interstitial fluid, may be pitting or non-pitting.
What is edema?
An appearance of yellowing of the skin or eyes.
Up to 1cm, contains fluid, raised, small and superficial. Examples include chicken pox and shingles.
What is a vesicle?
A mix of raised bumps and areas of discoloration.
What is Maculopapular rash?
Skin sheds 30,000 cells in one of these.
What is a minute?
When skin does not return to normal after being pinched.
What is tenting?
Bluish or grayish color of the skin, nails, lips or around the eyes.
What is cyanosis?
Larger than 1cm, solid, elevated. Examples include melanoma and hemangioma.
What is a Nodule?
A secondary skin lesion caused by a ruptured vesicle leading to non-bleeding loss of superficial dermis.
What is erosion?
19 million skin cells are in one square of this common measurement.
What is an inch?
The ABCDE's used to differentiate a mole from possible melanoma.
What are Asymmetry, Border, Irregularity, Color, Diameter and Elevation?
Bruising that may be caused by medical conditions or illness, rather than physical illness.
These result form changes over time caused by disease progression or manipulation such as scratching, picking or treatment.
What are secondary skin lesions?
Sign of long-term oxygen deprivation in the nails.
