1. Any drug and/or alcohol offense issued while operating a CMV.
2. Any Violation of a state or local law relating to motor vehicle traffic control (other than parking, vehicle weight, or vehicle defect violations) arising in connection with a fatal collision.
3.Any DOT collisions as defined by Safety Policy resulting in Fatality to another.
4. Leaving the scene of an accident
5. Assault with a motor vehicle.
What happens if the driver self discloses a MAJOR violation, and it is NOT reported on the VOE?
This needs to be investigated starting on the front end if it is on the application. We need to know what happened.
What should you tell the driver they need to send in if he has ran under his own authority?
*It is required that we contact any employer/carrier that you have been employed with in the last 3 years. For self-employment/owner-operator type employment, “ownership” documents need to be submitted. This includes tax documents for the period of self-employment/owner-operator status. In addition, a drug consortium, and 3 business references are required to ensure that your experience is verifiable. *
If a VOE comes in with a Pocily violation/ safety violation.... what is your FIRST steps?
Call the companys safet department to get additional information - if they don't prove it Ask Melissa Davis for permission to obtain a statement. (and wait for the approval once you send the statement in to Davis)
Should the driver follow up with the state for the Self Certification?
YES- We send it as a Curtsy and they are responsible to be sure the state files the card.
