Yes, in the e-charts folder there are instructions for scanning to a transitioned youth's e-chart.
When is the only time a parent can legally sign for a youth who is 18+?
If the youth's caregiver has guardianship (usually in DD cases) and it is scanned to the e-chart.
How long are psychiatric evaluations/notes good for?
one year
If a youth is transitioned, can I just write "youth tisp" on the review for any negatives?
No, only the CYBER section and consents can't be updated once a youth has transitioned. Missing paperwork will still need to be accounted for or deemed as not completed.
Who is required to be under formal supports.
All current CMO authorized services should have a contact name and number in the supports tab on CYBER.
If I have a 2 part CFT meeting, how many sign in sheets and ISP's are completed?
2 sign-in sheets (1 for each meeting) and 1 ISP with the submission date of the 2nd meeting.
Do the parents names need to be listed on the ROI for an 18 year old youth if we are involved with the parent as well?
Yes, unless the youth specifically signs off for communication with the parent on the ROI we are not allowed to speak with them.
How long are psychological assessments good for?
3 years
Can the assessments section be submitted as a negative?
yes, but an explanation is needed in the comments section.
I got a negative for youth contact info for a 13 year old youth. Can I just put N/A on the chart review for submission?
No. You need to input N/A - phone and email on the demographics tab in CYBER.
If my youth is DD, do they need to sign the ISP or CFT sign-in sheet?
If the youth is under 18 and not able then no. If the youth is 18+ and has no legal guardian then yes they need to at least make a mark.
Does a private attorney need to be listed under other or are they covered under the legal section?
Private attorneys need to be specifically listed on the ROI under other. Only the public defenders are covered under the legal umbrella that is in section 1.
What do I do if my youth does not have any assessments or the parents do not want to provide them?
You can write N/A - parent refused to provide or youth not on medication.
Is there any further steps to take once my chart review is submitted and approved?
Yes, you will get an email that you can bring any documents up to the review date to QAPI for shredding.
Is a resource parent considered an informal support?
Yes.
Does there need to be an SNA in my e-chart for each CFT meeting?
No. As long as the SNA is in CYBER, it is not required to be in the e-chart.
What do you do when a youth is 14+ and refuses to sign consents?
DO NOT write youth refused to sign. You will need to go home and try to engage with the youth another time.
Where do assessments such as neurodevelopmental or social assessments get filed?
In the youth's e-chart under section 2.3 (assessments > other)
What if my review is for a transfer youth from another CM and I do not have the documents requested?
You will need to contact the previous CM (if possible and still with COC) and ask about the paperwork. If the previous CM is not an option please speak with the CM's supervisor. Once that is completed, you will document your efforts in the comments section of the review.
