Each arrow is infested with a virus, if the virus deems you worthy it will *unlock* your stand and any wounds received from the arrow will instantly be healed. If you are not worthy then you will simply die from the wounds.
( Fun Fact: Josuke was able to help manifest Koichi's stand by healing his wounds before they killed him.)
