These people help individuals navigate an array of treatment options, behavioral healthcare programs, and community resources. They coordinate care to make sure individuals receive the appropriate treatment and customized care. They also connect individuals with available community resources, such as Social Security benefits, housing assistance, vocational training, employment, and healthcare.
Who are Case Managers?
Case Managers monitor treatment to help individuals on their path to mental wellness. Like advocates, they review treatment plans and assess goals to make sure individuals receive the highest quality of services.

This part of the agency provides a wide array of services from 24-hour supervision to weekly visits. We help individuals live independently in the community by offering skill-building services, training, and monitoring. For example, we support people with cooking, cleaning, budgeting, and getting along with neighbors.

This 4 letter program aims to keep people with mental health diagnoses These intensive services help individuals connect to the resources they need to avoid homelessness, incarceration, and hospitalization. The team includes mental health specialists, a psychiatrist, nurses, clinicians, substance and vocational specialists, and a peer specialist.
What is PACT?
Our Program for Assertive Community Treatment team meets individuals where they are and works with them around-the-clock. Some services include: managing medications, keeping appointments, finding a job, finding affordable housing, and learning daily living skills.
