Why do I need a crown and not a filling?

It is the strength of the tooth that holds a filling. Not the filling that holds the tooth.

 Once the filling becomes too large it needs a crown to add strength back.

What does it mean when insurance denies coverage due to attrition?

A dental insurance denial for "attrition" means that the insurance company is refusing to cover a procedure because the primary reason for the tooth damage is excessive wear and tear from normal chewing, which is considered "attrition" and is usually not covered under most dental insurance plans;

They view the damage as wear and tear rather than a necessary repair due to decay or fracture.
