In the scope of work "If the family cannot be reached by telephone after three (3) attempts at different times of the day, visit the family’s home in person."

30 Days.

Place eyes on the child(ren) by being in the same room as the child(ren) at the home visit in order to consider this visit completed.

If the Participant has lived outside of Arizona within the past 5 years.

KN - During the course of the referral, how many home visits are required?
Conduct home visits monthly for the first six (6) months and quarterly thereafter. Add more frequent visits as necessary based on the needs of the family and child.
What information should be in a calendar invite?
Participant name, service type, date/ time, add the KSS INBOX, Participant, Lead. Include the Address and phone number incase of coverage.

How should you move forward If a birth parent moves into the home during the process of conducting the Home Assessment?
KSS Specialist should finish the Home Assessment and document in the assessment that DCS is responsible for the safety assessment of the birth parent.
How many days prior to expiration of a licenses, should the KSS Specialist send the families a notice of license expiration and license renewal?
At least 90 calendar days prior to the expiration.
GAR - When should the Exhibit R be uploaded into Guardian?
Pinnacle:
30 Days after the case is assigned to the KSS Specialist.
20 Days for the Specialist to complete Home Visit and Exhibit.
10 Days for management to approve and upload into Guardian.
Scope: Upload the report to Guardian and submit to the Courts no less than fifteen (15) business days prior to the court hearing.
CS - What is the minimum time that you should be at the home visit for a Courtesy Supervision?
30 minutes minimum, if there is multiple children in the home, plan 15 minutes interview time per child.
