This is the primary document for all absences. The type(s) of leave and specific dates are completed on it. This document must be signed by the employee and supervisor. If an employee fails to submit this form the school should complete it and indicate "employee unavailable".

What is a Leave Request Form?

This type of leave can be taken for an illness of the employee or their immediate family. It is keyed at the worksite unless being processed by HR-leaves.

This form is  necessary when an employee is going out on medical leave and would like to receive donated leave. This form is completed by the person receiving the leave.

This type of leave can only be used with supervisor's approval and is not available for 10 month instructional personnel on days in which students are in session except if the absence is for Parental Leave or if it has been designated as a Catastrophic Illness by Human Resources.

This type of leave is available to cover an eligible employee's absences due to their own medical leave. Medical documentation is required, all sick and available annual leave must be exhausted before it is applied. Mandatory $50 deductions for each day it is used.

What is the employee must have been continuously employed for 1 year with a NC State agency and must have been in a paid status at least 1,040 hours in the 12 month period immediately prior to the qualifying event.

This is a form that is completed by Principals when an employee is returning back to work and they have been in a non paid status for more than 10 days.

Classroom teachers .02 days of this type of leave each pay period.  It can be used in whole or half day amounts. As long as a reason for using the leave is designated there is no deduction for using it, otherwise there is a $115/per day deduction.
