The OTA let the client know that the lunch tray was in her room so the client returned to her room and fed herself

When donning his shift, the client needed a little help to bring the shirt around his back and line up the first button.

When reading a bus route map, the OTA provided the client a legend that was simplified to support understanding. Client was able to read/comprehend the map using this legend.
What are visual cues or minimal visual cues.
During a toothbrushing task, the client could not put the paste on the brush, manipulate or hold the brush, but she did open her mouth, rind, and spit on command.
What is maximum assistance
The OTA noted that after the containers were opened and the food was cut, the resident was able to feed herself.
What is Independent with set-up.
The client who has chronic obstructive pulmonary disease unloaded the dishwasher, but needed several rest breaks in order to complete the task.
What is modified independence.
The child needed help for about 1/2 of the shoe-tying task.
What is moderate assistance.
A student is able to participate in an activity which has 3 steps, completing 2/3 steps with no verbal cues.
During craft group, because sharp objects were present, the OTA sat next to the client who was suicidal.

The student demonstrated ability to use her power wheelchair well, so the OTA put a smiley face sticker on the wheelchair.

Client is able to brush hair after being cued to go to the bathroom/mirror and finding brush on sink surface (that had been set there by OTA)

While the client was cooking at the stove, the OTA put an arm lightly around the client's back in case the client became unsteady.

The client with left neglect could read the newspaper article only after the OTA put a red line at the left margin.

During mealtime, the OTA had to touch the client's arm a few times to prompt the client to bring food to mouth.

The client could only remember to take her medicine when her cell phone timer buzzed.

Betty utilizes an orientation board to remember the date and to track activities for the day.
