The level of observation a patient in seclusion is on
What is Constant Awareness?
Every hour
What is the documentation time on the SA flow sheet?
You can watch up to 3 patients at one time on this level of observation
What is Special Awareness?
Is the involuntary confinement of a patient
What is Seclusion?
Every 15 minutes
What is the documentation time on the CA and CO flow sheet?
I require you to be no further than your arm's length away from a patient
What is Constant Observation?
The flowsheet used for a patient in Seclusion
What is a Seclusion flow sheet?
We require you to see both the hands and neck of the patient when you are on status with them
What is Constant Awareness and Constant Observation?
I should be writing the reason for seclusion on the flow sheet
Who is the Nurse?
The patient is watched when awake but not when asleep
What is SA/SO?
If safe to do so, I must be provided to a patient who is in the ITA for an extended time
