This is a document created at the beginning of a project to define the project's purpose and goals.
Organizational charter / project charter
Tool created in the Measure Phase for time related problems. This graph resembles a line chart and features both a good arrow and goal line.
Run chart
Tool created in the Analyze Phase and helps us get to an actionable root cause by showing cause and effect relationships. It features four major "types" of problems: machine, material, people, and process.
Fishbone Diagram
Tool used in the Improve Phase to brainstorm solutions that will reduce or eliminate the main root causes of a problem. This tool involves each person in a group taking turns sharing actionable solutions.
Round Robin
This is the purpose of the Control stage.
Maintain the gains/make sure the solution is actually working and people are following through on it
This is a diagram created in the Yellow Belt define stage to understand the suppliers, inputs, process, outputs, and customers involved in a process.
SIPOC Diagram
Tool created in the Measure Phase for quality related problems. This graph resembles a bar graph where each bar represents a different category.
Pareto chart
This is what you put at the head of a fishbone diagram.
Problem Statement
Tool used in the Improve Phase to help a team reach consensus (mutual agreement) about choosing a solution or course of action. It features 4 or 5 columns where we assign numbers to our solutions based on certain criteria.
Prioritization Matrix
Part of the Control Phase that helps us make sure that moving forward, people will continue to implement the change we have made in the process through our improvement project. In other words, our new solution needs to become the ONLY way of doing things.
Standardization
Customer opinion that comes to you whether you take action or not.
Reactive VOC
Uncontrolled or unexpected differences in a process's outputs.
Variation
This is the chart we have not used a lot, but features columns representing the people involved in a process. In those columns, the steps of a process are listed chronologically in either a circle, square, or diamond shape.
Flow chart
A method for systemizing a workspace to make it organized, clean, uncluttered, and safe, which can help reduce waste and optimize productivity.
5 S
What should a team do if they notice the solution is not working during the "checking results" step of the Control stage?
Go back to the improve stage and try to find a new solution
Customer opinion that you need to put effort into gathering.
Proactive VOC
Run charts with upper and lower control limits and spec limits.
Control chart
What do we call the type of work that wastes time and does not directly contribute to the end goal of the process?
Non-value added work
This is sometimes considered the 6th S of 5 S.
Safety
This is an example of a visual management tool.
Labels, posters, checklists, etc.
A tool that organizes the VOC (what customers want) into related groups.
Affinity Diagram
Graph that shows the shape or distribution of data by showing how often different values occur. Visualizes variation.
Histogram
A visual tool that examines your flow chart and tells you where the waiting is taking place in the process (non-value added work).
Value Stream Mapping
Designing the new and improved process so mistakes are impossible.
Mistake Proofing
Helps ensure sure everyone can follow the new process moving forward by creating and posting visual aids in the workspace.
