How do managers request PTO?

A PTO form should be completed at least two weeks in advance and sent to Coordinator, HR, and Payroll.

How long do you have to review GER's?

Three days, unless its an emergency GER. Emergency GER's need to be reviewed and sent to case manager within 24 hours of the incident date.

Once a year and should be brought to Instant Fire.

Employees are required to give at least two hour notice before the start of their shift and should call out to their manager or on call manager on the weekends. (CALL NOT TEXT)

Once a month and anything over a $50 value or sentimental value. All electronics should also be on it with the serial number.

Narcotics should be mixed with kitty litter or ground coffee while the manager and a staff are present. It should then be documented in the Narc count book and in the medication disposal record.
