Hypocortisolism:

ADH is a hormone involved in water reabsorption in the kidney. Glucocorticoids inhibit ADH secretion and thereby prevent excessive water retention.  With adrenal insufficiency, ADH secretion is disinhibited causing increased reabsorption of free water with subsequent dilution hyponatremia.
