Youth and staff must complete a detailed Aftercare Plan which includes: The amount of financial assistance being provided, and the specific reason for it,Specific activities the young adult will complete to achieve self-sufficiency or transition to EFC or PESS.
The Aftercare Rule requires: Staff to assess the young adult’s needs at the initial application and every three months thereafter. When financial assistance is provided, payments are made directly to the vendor until the youth and staff agree that the youth is capable of managing the money.
